Contact Information

It is very important that contact information on your account stay current to ensure that bills and other communications reach you in a timely manner.  Use this form to update phone number, email address and mailing address. Submit the Contact Information Update form via email , postal mail or in person.  

Name Changes

Changes to names currently on the utility account require additional documentation such as a divorce decree or marriage license.         Submit the Contact Information Update form via email , postal mail or in person. 

Adding an Occupant

This form may also be used to add an occupant to an existing water account.  Accounts may only have two names.  Both parties are considered to have financial responsibility for the account.  In addition to the form, a copy of valid U.S. state issued identification is required. The request to add an occupant must come from the person currently listed on the account. Submit the Contact Information Update form via email , postal mail or in person.
